WORKERS' COMPENSATION
PRIOR TO A LOSS, BE SURE THAT ALL CONTRACTS ARE PROPERLY EXECUTED AND ALL ASSUMED LIABILITIES ARE PROPERLY COVERED. BE SURE ALL SUBCONTRACTORS HAVE THE PROPER COVERAGES.
When an employee is injured, proper forms need to be completed and sent to various entities. Copies should be with the claims kit we provided; if not call for a copy. They are:
1. Texas Workers' Compensation
- TWCC-1 Employer's First Report of Injury
Others that may be required:
- TWCC-2 Employer's Report for Reimbursement of Voluntary Payments
- TWCC-3 Employer's Wage Statement
- TWCC-4 Employer's Contest of Compensability
2. United States Longshore and Harbor (USL&H)
- LS-202 Employer's First Report of Injury
3. Report all information as soon as possible to The GEM Agencies at 713-622-2330 or the company direct reporting number if applicable.
4. If the injury occurs in a state other than Texas, you will need to use that state's form, call GEM or the company for instructions.
